Rely Rewards Terms & Conditions

The Rely Rewards Loyalty Program (the “Program”) sponsored by Rely Medical Supply, LLC. (“Rely Medical”) is designed to help you get the most out of your membership. We reward you for doing the things you already do: shop online on RelyMedical.com (the “Site”), refer friends to RelyMedical.com, and share products and purchases. Best of all, you can use the points you earn to purchase more products online!

The following terms and conditions (these “Reward Program Terms and Conditions”) contain important information regarding the Program. Please read the following Rely Rewards Program Terms and Conditions carefully. Acceptance of the Site Terms and Conditions includes acceptance of these Reward Program Terms and Conditions. These Reward Program Terms and Conditions are a binding agreement between you and Rely Medical and will govern your participation in any and all Program offers.

Participating in the Program entitles members (“Members”) to take advantage of certain Program rewards (“Rewards”) that may change from time to time and, which may be made available for limited times, all as determined by Rely Medical in its sole discretion. Rely Medical reserves the right to modify these Rely Reward Program Terms and Conditions at any time, without notice to you, so it is important to check the Rely Reward Program Terms and Conditions periodically. Participation in the Program or redemption of Rewards is considered acceptance of these Rely Reward Program Terms and Conditions and any modified terms included therein. Upon notice, Rely Medical may, in its sole and absolute discretion, cancel, change, suspend or modify any aspect of the Program or any Program Reward at any time, including the availability of any Program Reward.

Program Period

This Program will continue until terminated, suspended, modified, or converted to another rewards program by Rely Medical (the "Program Period").

Eligibility

To become a Member of the Program, you must be a legal resident of the forty-eight (48) contiguous United States and the District of Columbia, and be 18 years of age or older.

Earning Rely Rewards Points

Once you become a member, you’ll start collecting Rely Rewards (“points”) every time you interact with us:

Registration — Earn 400 points for signing up for the Program (creating an account in our store).
Shop — Collect 5 points for every whole $1 spent on full-priced (non-clearance)* products from our online shop (the “Shop”) (excluding tax, shipping charges and fees).
Celebrate — Enjoy 1000 points on your birthday!
Sharing on social media — Want a little bonus for staying connected? Like us on Facebook and you'll receive 100 points.
Refer friends — Earn additional coupons for yourself and for every friend who signs up for Rely Medical.

* Exclusions: clearance items, sale products, special orders, discontinued items, and BOGO offers are not eligible for point earning or redemption.

Points will be available for use immediately after the applicable purchase.

Upon notice, Rely Medical reserves the right to suspend, terminate, revalue, or modify, without liability to Members, all or part of the points’ value structure and offers and any merchandise or service. Rely Medical reserves the right to adjudicate all points discrepancies in its sole discretion, and the Members agree to abide by any such adjudication. When you are logged in, you can always see your Rely Rewards points balance on any page of our site, along with the loyalty page.

Rely Rewards Points Redemption

The points are redeemable towards online purchases of full-priced products (non-clearance) on the Site in any increment. Points purchases must be made online on the Site.

You can redeem your points in a couple of ways. When you are logged in and have enough points to buy a product, a “Redeem your Points” box will appear at the top of the checkout page. Just choose your discount from the drop-down box, confirm your shipping info, click “Buy,” and you’re all set!

You can also redeem your points once logged into your account by clicking the Rewards box on the bottom corner of any store page to review your rewards details. It will show your points balance along with your options to redeem. Once you choose your discount, click on the arrow and a discount coupon code will appear for you to copy and apply at checkout, or you can click the 'Apply code' button to apply your discount to the purchase.

500 Points are equal to $1 of merchandise purchase.

Refunds & Returns

Any items received as a result of points redemption may only be exchanged for a refund of the applicable points redeemed for such item (not cash or credit). Purchases made with points will ship in the normal course of business.

Rely Rewards Points Minimums And Maximum

You cannot redeem points for more than the total amount of your purchase. Points are not redeemable for cash.

Rely Reward Points Balance

When you are logged in, you can always see your Rely Rewards points balance on any page of our site.

Rely Rewards Points Expiration

Your points remain valid for up to two years from the date they’re earned. We’ll even send you reminder emails 30 days and 3 days before any of your points are due to expire — so you’ll always have the chance to use them before they’re gone. If points are not used within that time frame, they will automatically expire and be removed from your balance.

Rely Medical reserves the right to modify, suspend, or cancel any part of the Rewards Program at any time without prior notice. We also reserve the right to resolve any points discrepancies at our sole discretion. By participating in the program, you agree to these terms. When you’re logged in, you can view your Rely Rewards Points balance on any page of our website. Start earning and saving today — it’s our way of saying thank you!

If Member does not redeem Points within this time frame, Member forfeits all such points.

Termination

Rely Medical may, in its sole and absolute discretion, cancel, change, suspend, or modify any aspect of the Program and/or any Reward at any time, including the availability of any Reward. Rely Medical may, in its sole and absolute discretion, terminate or suspend any Member’s participation in the Program for breach of these Rely Rewards Program Terms and Conditions, or for taking any actions that are inconsistent with the intent of these Rely Rewards Program Terms and Conditions. Rely Medical reserves the right at its sole discretion to prohibit any Member from participating in any aspect of the Program if Rely Medical deems or suspects that such Member has engaged in or has attempted to engage in any of the following: (a) acting in violation of these Rely Rewards Program Terms and Conditions; or (b) damaging, tampering with or corrupting the operation of the Program or Site; or (c) acting with intent to annoy, harass or abuse any other person; or (d) any inappropriate, uncooperative, disruptive, fraudulent, potentially fraudulent, or unusual behavior or activity; or (e) activity deemed in the sole discretion of Rely Medical to be generally inconsistent with the intended operation of the Program. Any decision Rely Medical makes relating to termination or suspension of any Member’s participation in the Program shall be final and binding in all respects. Rely Medical shall be the sole determiner in cases of suspected abuse, fraud, or breach of these Rely Rewards Program Terms and Conditions or intent of these Rely Rewards Program Terms and Conditions.

Release

Except for willful misconduct or gross negligence on the part of Rely Medical, by participating in the Program, Members release Rely Medical, its parent company, subsidiaries, affiliates, suppliers, advertising and promotions agencies and their respective directors, officers, employees, and agents (collectively, “Released Parties”) from any and all liability for any loss, harm, damages, cost or expense, including, without limitation, property damages, personal injury and/or death, arising out of or in any way connected to the Program or the use of any Rewards.

Indemnification

Members agree to indemnify, defend and hold Rely Medical and its representatives and agents harmless from and against any and all third party claims, demands, liabilities, costs or expenses, including attorney’s fees and costs, arising from, or related to any breach by Member of any of these Rely Rewards Program Terms and Conditions or any violation by Member of applicable law.

Governing Law

The laws of the State of Minnesota shall govern these Rely Rewards Program Terms and Conditions. Member hereby expressly consents to exclusive jurisdiction and venue in the courts located in Minnesota for all matters arising in connection with these Rely Rewards Program Terms and Conditions or Member’s participation in the Program.
