Rely Rewards Program Terms and Conditions

The Rely Rewards Loyalty Program (the “Program”) sponsored by Rely Medical Supply, Inc. (“Rely Medical”) is designed to help you get the most out of your membership. We reward you for doing the things you already do: shop online on RelyMedical.com (the “Site”), refer friends to RelyMedical.com, and share products and purchases. Best of all, you can use the points you earn to purchase more products online! The following terms and conditions (these “Reward Program Terms and Conditions”) contain important information regarding the Program. Please read the following Rely Rewards Program Terms and Conditions carefully. Acceptance of the Site Terms and Conditions includes acceptance of these Reward Program Terms and Conditions. These Reward Program Terms and Conditions are a binding agreement between you and Rely Medical and will govern your participation in any and all Program offers.

Participating in the Program entitles members (“Members”) to take advantage of certain Program rewards (“Rewards”) that may change from time to time and, which may be made available for limited times, all as determined by Rely Medical in its sole discretion. Rely Medical reserves the right to modify these Rely Reward Program Terms and Conditions at any time, without notice to you, so it is important to check the Rely Reward Program Terms and Conditions periodically. Participation in the Program or redemption of Rewards is considered acceptance of these Rely Reward Program Terms and Conditions and any modified terms included therein. Upon notice, Rely Medical may, in its sole and absolute discretion, cancel, change, suspend or modify any aspect of the Program or any Program Reward at any time, including the availability of any Program Reward.

Program Period

This Program will continue until terminated, suspended, modified, or converted to another rewards program by Rely Medical (the "Program Period").

Eligibility

To become a Member of the Program, you must be a legal resident of the forty-eight (48) contiguous United States and the District of Columbia, and be 18 years of age or older.

Earning Rely Rewards Points

Once you have become a Member, you can begin accumulating Rely Reward points (“Points”) the following ways:

Registration — earn 20 points for signing-up for the Program (setting up an account in our store).
Shop — every time you purchase a full-priced (non-clearance)* product from our online shop (the “Shop”), you earn 1 Point for every whole $1 spent per order (excluding tax, shipping charges and fees). If you purchase a full-priced (non-clearance)* Rely branded product, you earn 1.5 Points for every whole dollar you spend per order!
Refer friends — earn 20 Points for every friend who signs up for Rely Medical.
Sharing on social media — earn 1 Point per week for each share you make on social media via Facebook or Twitter.

* Exclusions: clearance, sale items, special orders, discontinued, and BOGO products are excluded from point redemption.

Points will be available for use 45 days after the applicable purchase.

Upon notice, Rely Medical reserves the right to suspend, terminate, revalue or modify, without liability to Members, all or part of the Points’ value structure and offers and any merchandise or service. Rely Medical reserves the right to adjudicate all Points discrepancies in its sole discretion, and the Members agree to abide by any such adjudication. When you are logged in, you can always see your Rely Rewards Points balance on any page of our site.

Rely Rewards Points Redemption

The Points are redeemable towards online purchases of full-priced products (non-clearance) on the Site in any increment. You can redeem your Points from the product page, before adding a product to your cart. You may not use Points to purchase recurring orders. Points purchases must be made online on the Site.

When you are logged in and have enough points to buy a product, a "Buy with Points" button will appear next to the “Add to Cart” button. When you select "Buy with Points" you will be asked to confirm/change your shipping info. Once you confirm the shipping info., select "Buy" and you're done!

When redeeming Points, you may only “buy” one product at a time.

20 Points are equal to $1 of merchandise purchase.

Refunds & Returns

Any items received as a result of Points redemption may only be exchanged for a refund of the applicable Points redeemed for such item (not cash or credit). Purchases made with Points will ship in the normal course of business.

Rely Rewards Points Minimums And Maximum

You cannot redeem Points for more than the total amount of your purchase. Points are not redeemable for cash.

Rely Reward Points Balance

When you are logged in, you can always see your Rely Rewards Points balance on any page of our site.

Rely Rewards Points Expiration

Points will expire 3 years from the date they were first earned. If Member does not redeem Points within this time frame, Member forfeits all such Points.

Termination

Rely Medical may, in its sole and absolute discretion, cancel, change, suspend, or modify any aspect of the Program and/or any Reward at any time, including the availability of any Reward. Rely medical may, in its sole and absolute discretion, terminate or suspend any Member’s participation in the Program for breach of these Rely Rewards Program Terms and Conditions, or for taking any actions that are inconsistent with the intent of these Rely Rewards Program Terms and Conditions. Rely Medical reserves the right at its sole discretion to prohibit any Member from participating in any aspect of the Program if Rely Medical deems or suspects that such Member has engaged in or has attempted to engage in any of the following: (a) acting in violation of these Rely Rewards Program Terms and Conditions; or (b) damaging, tampering with or corrupting the operation of the Program or Site; or (c) acting with intent to annoy, harass or abuse any other person; or (d) any inappropriate, uncooperative, disruptive, fraudulent, potentially fraudulent, or unusual behavior or activity; or (e) activity deemed in the sole discretion of Rely Medical to be generally inconsistent with the intended operation of the Program. Any decision Rely Medical makes relating to termination or suspension of any Member’s participation in the Program shall be final and binding in all respects. Rely Medical shall be the sole determiner in cases of suspected abuse, fraud, or breach of these Rely Rewards Program Terms and Conditions or intent of these Rely Rewards Program Terms and Conditions.

Release

Except for willful misconduct or gross negligence on the part of Rely medical, by participating in the Program, Members release Rely medical, its parent company, subsidiaries, affiliates, suppliers, advertising and promotions agencies and their respective directors, officers, employees, and agents (collectively, “Released Parties”) from any and all liability for any loss, harm, damages, cost or expense, including, without limitation, property damages, personal injury and/or death, arising out of or in any way connected to the Program or the use of any Rewards.

Indemnification

Members agree to indemnify, defend and hold Rely Medical and its representatives and agents harmless from and against any and all third party claims, demands, liabilities, costs or expenses, including attorney’s fees and costs, arising from, or related to any breach by Member of any of these Rely Rewards Program Terms and Conditions or any violation by Member of applicable law.

Governing Law

The laws of the State of Minnesota shall govern these Rely Rewards Program Terms and Conditions. Member hereby expressly consents to exclusive jurisdiction and venue in the courts located in Minnesota for all matters arising in connection with these Rely Rewards Program Terms and Conditions or Member’s participation in the Program.
